›› Recommended Immunizations and Tests at Each Checkup

Complete physical exams are recommended at 2 weeks, 2, 4, 6, 9, 12, 15, & 18 months and annually from ages 2 - 6 years. After the age of 6, physical exams are recommended every two years unless your child has a chronic health issue (such as ADD or Asthma or needs medications prescribed). Those children need yearly checkups. And annual exams are sometimes necessary to provide medical information required by some camp and school forms.

Immunization Schedule
2-4 Days Exam, with special attention to weight and color (jaundice)
2 Weeks Exam, weight check and parent questions
1 Month Hepatitis B (unless planning to use Pediarix)
2 Months DTaP, HIB, IPV, Prevnar  OR Pediarix, HIB, Prevnar
4 Months DTaP, HIB, IPV, Prevnar  OR Pediarix, HIB, Prevnar
6 Months DTaP, HIB, Prevnar OR Pediarix, HIB, Prevnar
9 Months Hepatitis B (if not using Pediarix), Denver Developmental Screen
12 Months IPV, Prevnar, Hgb, (and, if indicated, TB skin test, lead screen)
15 Months MMR, HIB
18 Months DTaP, Varivax
2-4 Years Lead Screen (if at risk)
4 Years Hgb, urinalysis (can optionally give one or all of 5 year shots)
5 Years DTaP, MMR, IPV, urinalysis, vision and hearing screens
6-9 Years Urinalysis. Other screening tests may be discussed based upon family history (for example: Cholesterol)
10-21 Years As indicated: Cholesterol, Hgb, MMR, Tetanus boosters and Hepatitis B series if not received earlier
