What is a medical home?

Our goal is to take care of children and adolescents in a thoughtful,comprehensive way until early adulthood. To that end, we offer easily accessible office hours and same day sick appointments. We have sick office hours on weekends and holidays and provide triage services at night and on holidays when we are closed with no additional change.


In addition to making sure that we can see our patients in a timely manner, a critical part of the equation is regular wellness visits. At these visits, our team monitors growth and development, addresses ongoing medical concerns, and assesses vaccine status.. At this time, we can make referrals to specialists, discuss lifestyle changes, or complete sports physicals. In short, some of the concerns we address can help prevent worsening of chronic conditions, and even prevent some sick visits! The most rewarding part of all is that we get to know the patients and their families better as people-- learning more about their community, their strengths, and their needs.

Behind the scenes, we are constantly working to build a network to help connect our patients to needed services such as nutritionists, therapists, and community resources. We also work to make sure that we receive necessary medical records and laboratory results.

We ask you to choose only one medical home where you will seek medical care routinely. Urgent care is not enough. It is essential to see our patients for both wellness and sick care to deliver the best care possible. Seeing one provider for well care and another for sick care results in fragmented care. We don’t know which medications or conditions other providers may have diagnosed and their reasoning.

Chapel Hill Children’s Clinic has gone through a rigorous national certification process to demonstrate our commitment to providing patient-centered care. We were recognized as a NCQA level 3 medical home, which is the highest level of achievement. We will continue to focus on providing high-value medical care through many programs as various health-care models emerge in coming years.

You may have heard news about rising numbers of measles cases in Texas and other areas of the United States. 2 doses of the MMR vaccine are highly effective (97%) at preventing measles infections. At CHCAC, children receive their first dose at age 1 and a second dose at age 4, to make sure kids are fully protected as soon as possible, and following the recommendations from the CDC. Some infants aged 6 months to 11 months who travel internationally to high-prevlance areas may need a dose to protect them, but they still need the 1 year and 4 year old doses as well. If you are ever concerned about a possible exposure to measles, please CALL before entering our office. DO NOT ENTER the office. This is a highly contagious illness and special precautions must be taken to prevent spread.

❄️ Winter Weather Update for Saturday 1/31/2026 ❄️ The office will be closed on Saturday morning for in-office sick visits to protect the safety of our patients and staff from the impending winter storm. Please call 919-967-0771 and press 1 to reach our nursing advice line. You can request a telehealth appointment through our portal; however, please note that it will not be continuously monitored. Please call the nurse advice line above for immediate assistance. If we receive the large amount of snow accumulation expected, we will not be able to open for Sunday morning sick clinic.
